Vital Bloom Telehealth, PLLC ("Vital Bloom," "we," "us") respects your privacy. This policy describes how we would collect, use, and safeguard information in connection with our telehealth services.

Information we collect

In a production service we would collect the information you provide when you create an account, complete an intake, book a visit, or contact us — including your name, contact details, and health information you choose to share. We would also collect limited technical data (such as device and usage information) to operate and improve the service.

How we use information

Information would be used to provide and coordinate your care, schedule and conduct secure video visits, communicate with you, and meet legal and regulatory obligations. Protected health information (PHI) is handled in accordance with HIPAA and our Notice of Privacy Practices.

How we protect information

A production launch would employ encryption in transit and at rest, access controls, audit logging, and signed Business Associate Agreements (BAAs) with vendors such as Meetkeep and our video provider. We would never sell your personal information.

Your choices

You would be able to access, correct, or request deletion of your information, and opt out of marketing communications at any time.
